there apparently is no "completed, booted Windows" code. However if I just boot normally without a trip through the BIOS then I get the D4. Been running my board for several months now and have had no problems at all with it. Setting the SB switch to single and then using the BIOS_SW to toggle between two different BIOS's like you mention. I have read on another manufacturers website that the A0 code is nothing to be concerned about, so some further clarification about the A0 code would be very much appreciated.
So when you see A0 you should just assume it's booted Windows.

So the solution was to disable dual BIOS mechanism straight up, either by a switch or through the UEFI, i think switch is better so you can re-enable it if one then is ok no?
